Medicare Part D Cost Calculator (2026 and 2027)
Enter your monthly drug costs to see what you'll pay each month, when you reach the cap, and how the payment plan would spread it out.
What you pay for Part D drugs in 2026 and 2027
Under the standard benefit with the full deductible ($615 in 2026, $700 in 2027) and 25% coinsurance, for the same drug costs every month.
| Drug costs a month | You pay in 2026 | Cap reached (2026) | You pay in 2027 | Cap reached (2027) |
|---|---|---|---|---|
| $100 | $761 | not reached | $825 | not reached |
| $300 | $1,361 | not reached | $1,425 | not reached |
| $600 | $2,100 | November | $2,325 | not reached |
| $1,500 | $2,100 | May | $2,400 | May |
The caps are $2,100 for 2026 and $2,400 for 2027; once you reach one, covered drugs cost nothing for the rest of that year. Plans with flat copays can come out lower, never higher.
The 2026 and 2027 Part D limits
Under the standard Part D benefit you pay the deductible, at most $615 in 2026 and $700 in 2027, then 25% of your covered drug costs until your out-of-pocket spending reaches the yearly cap: $2,100 in 2026 and $2,400 in 2027. After that, covered drugs cost you nothing for the rest of the year. With $600 a month of drugs you reach the 2026 cap in November; with $1,500 a month, in May. Many plans use flat copays instead of 25%, but none can make you pay more than the cap.
Spreading the cost with the payment plan
The Medicare Prescription Payment Plan lets you pay your plan in monthly bills instead of at the pharmacy. It doesn't lower the total. The first month's bill is at most the cap spread over the months left, $175 if you join in January 2026, and later bills are your remaining balance divided by the months left, so they rise as new costs arrive. Joining late in the year leaves fewer months to spread over, so it helps most for high costs early in the year.
Frequently asked questions
What is the Part D out-of-pocket maximum for 2026?
$2,100 for covered drugs, up from $2,000 in 2025, and $2,400 in 2027. It counts what you pay, including the deductible, and payments made for you through Extra Help. Once you reach it you pay nothing more for covered Part D drugs that year.
What is the Part D deductible in 2026?
At most $615, and $700 in 2027. Some plans have a lower deductible or none at all.
Should I join the Medicare Prescription Payment Plan?
It helps if you face large costs early in the year, because it caps what you pay each month and spreads the rest out. It doesn't save money overall, and if your costs are small or you join late in the year it makes little difference.
